Application of Hot Water Bag

Definition

Applying a hot water bag is a classic nursing intervention used to deliver localized dry heat for therapeutic purposes. It’s simple, effective, and when done correctly very comforting.

Purposes
  • To stimulate circulation by dilating blood vessels.
  • To relieve pain and congestion by encouraging flow of blood.
  • To supply warmth and comfort.
  • To promote healing.
  • To relieve retention of urine.
  • To relieve muscle spasm.
  • To reduce tissue swelling
  • To counteract sudden drop in temperature during cold sponging.
  • To raise body temperature in case of hypothermia.

Contraindications
  1. Acute inflammation, e.g., acute appendicitis and tooth abscess.
  2. Very young or very old patient.
  3. Patients who are unconscious or insane.
  4. Patients with sensorineural deficits, e.g., diabetes mellitus and peripheral neuropathy.
  5. Patients with high temperature.
  6. Open wounds.
  7. Malignancy.
  8. Blisters/burns

Procedure

 NURSING ACTIONRATIONALE
    1.Before procedure    Identify the patient and check the patient’s chart for any special instructions and nursing care plan.   
2.Provide an explanation to the patient regarding the procedure and the care to be taken.  Knowledge about the care during bag application will help prevent accidental burn injury.  
3.Assess for the presence of lotion or oil over skin surface at the site of application.Heat can be retained with the presence of these products and lead to increased risk of the intolerance and burns.  
    4.During procedure   Wash hands.    Prevents chances of cross- infection.  
5.Fill the hot water bag with hot water, secure the cap, and turn it upside down.  Ensures that there are no leaks and pre-warms the bag.
6.Empty the bag and refill with hot water (temp: 105-115°F) to about two-third full.  Filling to two- thirds level ensures comfort when applied.  
7.Place the bag on a flat surface, such as a table and expel all air by forcing the water up to the neck of the bag.  When air is removed, the bag is easier to mold over body part.  
8.Screw the cap tightly, dry outside of the bag using duster, and check it for leakage.  Prevents chances of scalding.  
9.Wipe off any moisture on the outside of the bag.  Moisture on the outside of bag increase risk of burn.  
10.Put on the cover and take to the bedside. If cover is not available, use pillowcase or towel.    Use of bag directly over the skin can cause burns. Thick covers can reduce effect of hot application.    
11.Apply to the area as ordered.   
12.Remove the bag after 20-30 minutes.    Keeping the bag for 20-30 minutes avoids chances of burn injury.
13.Check the site for redness, blisters,etc., if present apply Vaseline /oil.  Vaseline/oil soothes the skin.  
    14.After procedure   Make patient comfortable in bed.   
15.Record in the patients charts the time, site, duration of application, and observed.   
16.Take all articles to the utility room. Empty the hot water bag, wash outside of the bag with soap and water, and hang upside down to dry it. 

Special Considerations

  1. Do not leave the patient unattended with hot bag applied /in place.
  2. Never place an extremity on top of hot water bag as it can cause burn injury.
