Providing Sitz Bath

Fundamental Nursing Procedures

Definition

Providing a sitz bath is a gentle, noninvasive nursing intervention used to relieve discomfort and promote healing in the perineal and anal areas. It’s especially helpful after childbirth, anorectal surgery, or in cases of hemorrhoids, fissures, or infections.

Purposes

  1. To aid the healing process of perineal wounds
  2. To reduce pain and discomfort.
  3. To reduce swelling and irritation.
  4. To increase circulation.
  5. To promote relaxation

Indications

  1. Following surgery in the anorectal region.
  2. Follow Incision in the perineum, e.g., episiotomy
  3. Swollen painful hemorrhoids

Contraindications

  1. Diabetes, peripheral vascular diseases
  2. Impaired, peripheral sensory function.
  3. Immediate post-hemorrhoidectomy.

  1. Sitz bath basin
  2. Warm water (temp 105-110°F/48-43°C)
  3. Medication if ordered
  4. Bath thermometer
  5. Clean gloves (optional)
  6. Towel.
  7. Robe or sheet

Procedure

 NURSING ACTIONRATIONALE
  1.Before procedure    Check the physician’s order and nursing care plan 
2.Identify the patient.   
3.Assess the patient’s condition, pain level, and the ability to ambulate to the bathroom.  Patient who has taken pain medication may experience light-headedness or drowsiness impairing their ability to ambulate and tolerate the procedure.  
  4.During procedure   Wash hands.  Reduces transmission of microorganisms.  
5.Add required quantity of ordered medication, e.g., betadine into the sitz bath basin.   
6.Fill the basin with warm water at a temperature of 105-110°F w (40-43°C).  Warm water is soothing and results in vasodilation to enhance healing.  
7.Keep the basin on a low stool/potty chair/toilet bowl with toilet seat up.   
8.Assist patient with removal of any dressing or peri pad and position po and assist to sit in sitz bath basin.   Place a towel on the patient’s thigh and cover the shoulders with a sheet.  For postpartum patients, peripad must be removed from front to back to prevent contamination. Promotes warmth and prevents chills.  
9.Do not leave the patient unattended during the procedure. Instruct patient to contract and relax anal sphincter while taking sitz bath.  
10.Allow patient to sit for 20 minutes, then assist with drying and applying dressing or peripad as required.  Beneficial effects of heat are lost after 20 minutes due to vasoconstriction.  
11.Discontinue the procedure if any adverse effects like dizziness, weakness, accelerated pulse rate, or pallor occurs.   
12.Assist patient back to bed and instruct to stay in bed for 20 minutes with hip elevated.Use of warm water and prolonged sitting in one position may result in light headedness on arising.  
  13.After procedure   Clean and replace all the reusable articles. If the basin is to be autoclaved, arrange for it.   
14.Wash hands.   
15.Document tolerance of procedure, pain, and appearance of the perineal area/wound area.  Decrease swelling, redness and drainage and complete healing are the goals.

Special Consideration

If the patient has received epidural anesthesia, be sure that complete sensation has returned before using a warm sitz bath.
