Process recording is the interaction or interview conducted and recorded by the nurse by using various communication techniques.

Purposes
  • To study the patient’s psychological, social and emotional behavior.
  • To evaluate the condition of patient during admission, hospital stay, and at the time of termination of hospital stay.
  • To improve therapeutic communication skills of the nurse.
  • To establish a therapeutic relationship.
General guidelines
  1. Obtain patient’s permission for recording and using the conversation for therapy. This will help in reviewing the session if needed.
  2. Record the conversation verbatim.
  3. Note the nonverbal responses of patient and nurse during the recording the session.

Procedure

1. Preparation

  • Choose a quiet, private setting to ensure confidentiality.
  • Obtain patient consent before recording interactions.

2. Conducting the Interaction

  • Document the conversation verbatim, noting tone, pauses, and emotional expressions.
  • Include interpretations of the patient’s responses and possible underlying emotions.

4. Analysis and Reflection

  • Evaluate the effectiveness of communication techniques used.
  • Identify areas for improvement in therapeutic interactions.
  • Reflect on personal feelings and responses during the session.

5. Documentation and Review

  • Ensure the process recording is accurate, objective, and well-organized.
  • Discuss findings with supervisors or peers for feedback and learning.
